Flat Rate for Medical Treatment obtained at McLeod
The medical plan covers several common services and procedures with only a flat rate (copay) paid by the member if the service is received at a McLeod Health provider and facility. No additional deductible or coinsurance will be required by covered employees and their families for these services. Covered services include:
- Basic Labs: $50 copay
- X-rays and Ultrasounds: $100 copay
- MRIs and CT’s: $200 copay
- Maternity/birth related charges: $500 copay for mother’s inpatient stay and delivery

*Please note that the High Deductible plan is eligible for copay only program after deductible has been met. Deductible must be met first per HDHP rules.
