The 2024 Healthier You credit will be a one-time $100 payroll credit applied to your paycheck in April 2024. To receive the 2024 credit, employees and their spouses who are on the McLeod Health Insurance Plan, will be required to simply create a MyChart account and complete a Health Risk Assessment (HRA) between January 1, 2024 and March 31, 2024.
*New hire employees will be required to complete the HRA within 31 days of hire.
To access the Health Risk Assessment Questionnaire:
1. Login into MyChart
2. Select the “Menu” tab
3. Scroll down to the heading “My Record”
4. Select “Questionnaires” tab
5. Select “Health Risk Assessment” tab and complete questionnaire
No biometric screening will be required to receive the one-time $100 credit for 2024, but you will be required to complete the Health Risk Assessment (HRA) in MyChart.
If you have not already signed up for a McLeod MyChart account, visit www.McLeodMyChart.org and request a New User Account. Once your account is created, you’ll be able to log in to your MyChart via desktop or by downloading the app on your smartphone in the App Store or Google Play.
